Jay Pulitano, LMSW is a Behavioral Health Therapist. He treats patients at our Washington Heights Health Center. Jay completed his graduate degree at CUNY Hunter College. He completed his internship at Callen-Lorde Community Health Center.
Jay Pulitano takes an open-minded, inquisitive, trauma-informed, and client-centered approach in his work as a behavioral health therapist. He approaches his work through an anti-oppressive lens, taking into consideration the impact of racism, sexism, homophobia, transphobia, able-ism, and other oppressive systems on one’s behavioral health. Jay Pulitano uses a variety of techniques in
his clinical work, including bringing awareness to the impact of a patient’s personal history, teaching relaxation/mindfulness skills, and collaborating with a patient to identify and reshape the thought patterns that have a negative influence on emotions and behavior. Having 5+ years of experience serving the LGBTQ community in community health settings, Jay Pulitano is especially passionate about working with queer and trans individuals.
Jenny Mintz, LCSW is a Behavioral Health Therapist. She treats patients at our Williamsburg Health Center in Brooklyn. Jenny completed her Bachelor of Arts at Rutgers University. She completed her Master of Social Work at NYU Silver School of Social Work.
Geri Herisse, RDN, CDN is a board-certified Registered Dietitian Nutritionist. She treats patients at CHN Harlem/Helen B. Atkinson in Harlem, NY. Geri is fluent in English and Haitian Creole. She completed her undergraduate degree at Hunter College, CUNY, and subsequently completed her dietetic internship at Teachers College, Columbia University. In her spare time, she likes to cook, and travel and enjoys attending food tours and experimenting with different cuisines.
Josie Thiele, MS, RDN/CDN, CLC is a board-certified Registered Dietitian Nutritionist. She treats patients at multiple CHN sites but is based out of the Williamsburg Clinic. Josie is fluent in English. She completed her undergraduate degree at Colby College and earned a Masters in Nutrition from Boston University.
Nazish Pervez is a board certified Nutritionist. She treats patients at our Long Island City Health Center in Queens. Nazish Pervez completed her graduate degree at Queens College.
Keshia Okorie is a board-certified Registered Dietitian Nutritionist. She treats patients at our East New York Health Hub Health Center in Brooklyn, NY. Keshia is fluent in English. She completed her undergraduate degree at the City University of New York, Brooklyn College, and completed her dietetic internship at New York-Presbyterian Hospital. She completed a Fellowship in Outpatient Nutrition at New York-Presbyterian Hospital as well.
Bryan Tolentino, MD is a double-board certified physician in Internal Medicine and Infectious Diseases. He treats patients at our Harlem Health Center in Manhattan and Williamsburg Health Center in Brooklyn. He speaks English and Spanish fluently.
Dr. Tolentino earned his medical degree with Summa Cum Laude Honors from Pontificia Universidad Catolica Madre y Maestra. He completed his residency in Internal Medicine at Mount Sinai Medical Center and completed his fellowship in Infectious Diseases at Icahn School of Medicine at Mount Sinai St. Luke’s-Roosevelt Hospital Center.
Dr. Tolentino has a special interest in HIV medicine including PrEP and PEP, treatment of hepatitis B and hepatitis C, and LGBTQ+ care.
Colette Russen, FNP-C is a board-certified Family Nurse Practitioner. She treats patients at our East New York Hub Health Center in Brooklyn.
Colette completed her advanced practice nursing degree at Pace University, and completed Community Healthcare Network’s Family Nurse Practitioner Fellowship program. She completed her undergraduate nursing education at Phillips School of Nursing at Mount Sinai Beth Israel and Jacksonville University. She is credentialed as a specialist in HIV primary care by the American Academy of HIV Medicine.
