Dr. Pushpa Yadav, MD, is a board-certified family medicine physician who works as a faculty physician at Community Healthcare Network. She treats patients at our Long Island City location. Dr. Yadav is from Nepal and completed her medical degree from People’s Friendship University of Russia in 2002 and completed her family medicine residency at BronxCare Health System in 2024. Dr. Yadav is fluent in English, Nepali, Hindi, and Russian. Prior to her family medicine residency journey in the Bronx, she has worked for more than 10 years as a primary care physician across Nepal, American Samoa, and Marshall Island. She enjoys traveling, spending time with family and friends, and listening to music.
Frankie L. McLaurin, LCSW-R, PMHNP-BC is a Nurse Practitioner in Psychiatry who treats patients at the CHN Williamsburg Health Center. Mr. McLaurin completed an undergraduate degree in sociology at North Park University in 2008. He later completed his first graduate degrees in social work at New York University in 2012 and a second graduate degree in psychiatric-mental health nursing practice in 2023 at Yale University. He completed his adult clinical rotation at The Connecticut Mental Health Center from 2021- 2022, and his pediatric clinical rotation at The Yale Child Study Center in 2022-2023.
Jasmine Luciano, RD, CDN, is a board certified Registered Dietitian who works as a Nutritionist. She treats patients at our East New York/ Dr. Betty Shabazz Health Center. Jasmine Luciano completed an undergraduate degree in Health & Nutrition Sciences at Brooklyn College in 2023. She also completed an undergraduate degree in Culinary Arts at the Culinary Institute of America in 2016.
David Davis, DO is a board-certified Family Medicine Doctor of Osteopathic Medicine who works as a Medical Director. He treats patients at our Lower East Side Health Center. David Davis, DO graduated from Philadelphia College of Osteopathic Medicine in June 1972 and completed his residency from Tri County Hospital in June 1978.
Surong Li, MPH, MS, RDN, MCHES is a board-certified Registered Dietitian Nutritionist. She treats patients at our Lower East Side Health Center, Seward School-Based Health Center, and East New York Hub. Surong is fluent in English, Cantonese, and Mandarin. Surong completed her undergraduate degree in Nutrition and Food Science Dietetics at Hunter College, City University of New York (CUNY) in 2014 and completed her masters in in Social Behavior and Community Health at University at Albany, State University of New York (SUNY) in 2016 and Clinical Nutrition at New York University in 2024. She completed her dietetic internship at NewYork–Presbyterian Hospital.
Nadeen Eliyan is a Family Nurse Practitioner who works as a FNP fellow. She treats patients at multiple CHN locations. Nadeen completed an undergraduate degree in Child Development at California Polystechnic State University, San Luis Obispo in 2020 and completed her Family Nurse Practitioner degree at Columbia University in 2024.
Miriam Wagner, LMSW is a Licensed Social Worker who works as a Depression Care Manager. She treats patients at our Washington Heights Center. Miriam Wagner, LMSW completed an undergraduate degree in Psychology and Political Science at CUNY Queens College in 2019 and completed her masters in social work at NYU Silver School of Social Work in 2021.
Miriam Wagner works with clients to achieve the goals that matter to them including minimizing negative thoughts, breaking free from unhelpful behaviors, building confidence, and managing difficult emotions. Miriam works collaboratively with clients and utilizes evidence-based practices such as CBT, DBT informed, and mindfulness to help clients reach their goals.
Fabiola Alcime is a licensed Behavioral Health Therapist. She treats patients at our Health Centers in South Bronx. Fabiola Alcime completed her graduate degree at Stony Brook University, School of Social Welfare.
Dr. Ricketts is a board-certified Optometrist at Community Healthcare Network. She treats patients at our Washington Heights, Crown Heights, South Bronx, DBS, Williamsburg, and Long Island City Health Centers in the Optometry Mobile Unit.
Dr. Charis-Ann Ricketts graduated from the University of the West Indies with a BSc in Optometry. She then earned her Doctoral Optometry degree from New England College of Optometry in Boston where she completed clinical rotations through private practice, community health centers, and Ophthalmology practices. She completed a residency program in Primary Care specializing in Cornea and Contact Lenses at Nova Southeastern University.
Dr. Ricketts believes in a comprehensive and integrated approach to healthcare. Her sound clinical background and desire to provide high-quality care are evident in each patient encounter. She is proud to be part of an establishment dedicated to patient care, education, and creating optometric service platforms for sustainable vision care.
All Community Healthcare Network health centers and offices will be open on Tuesday, February 24, 2026. Our mobile health clinics will not be operational due to weather conditions.

We would like to inform you that as of September 1, 2026, access to your child’s Healow account through Community Healthcare Network (CHN) will be limited to viewing immunizations and referrals for all patients ages 12 and older. This change is required to comply with the Health Insurance Portability and Accountability Act (HIPAA) and New York State laws that protect minors’ rights to confidentiality.
You may still request your child’s medical records by calling or visiting your health center.
