Keshia Okorie is a board-certified Registered Dietitian Nutritionist. She treats patients at our East New York Health Hub Health Center in Brooklyn, NY. Keshia is fluent in English. She completed her undergraduate degree at the City University of New York, Brooklyn College, and completed her dietetic internship at New York-Presbyterian Hospital. She completed a Fellowship in Outpatient Nutrition at New York-Presbyterian Hospital as well.
Bryan Tolentino, MD is a double-board certified physician in Internal Medicine and Infectious Diseases. He treats patients at our Harlem Health Center in Manhattan and Williamsburg Health Center in Brooklyn. He speaks English and Spanish fluently.
Dr. Tolentino earned his medical degree with Summa Cum Laude Honors from Pontificia Universidad Catolica Madre y Maestra. He completed his residency in Internal Medicine at Mount Sinai Medical Center and completed his fellowship in Infectious Diseases at Icahn School of Medicine at Mount Sinai St. Luke’s-Roosevelt Hospital Center.
Dr. Tolentino has a special interest in HIV medicine including PrEP and PEP, treatment of hepatitis B and hepatitis C, and LGBTQ+ care.
Colette Russen, FNP-C is a board-certified Family Nurse Practitioner. She treats patients at our East New York Hub Health Center in Brooklyn.
Colette completed her advanced practice nursing degree at Pace University, and completed Community Healthcare Network’s Family Nurse Practitioner Fellowship program. She completed her undergraduate nursing education at Phillips School of Nursing at Mount Sinai Beth Israel and Jacksonville University. She is credentialed as a specialist in HIV primary care by the American Academy of HIV Medicine.
Brianna Bouchez, FNP is a graduate of Yale University’s Family Nurse Practitioner program with a concentration in diabetes management. Her work as a registered nurse includes acute care pediatrics, adult intensive care, and adolescent psychiatric care. Brianna holds a faculty position in a vocational nursing program, where she teaches in the classroom and clinical settings. She is an endorsed instructor for the Department of Children and Family Services, where she instructs residential care facilities on the principles of safe medication administration. Volunteering in Haiti was a pivotal experience in shifting her focus to primary care. Brianna is passionate about patient education and a career in the community health setting.
Alison Martin, DNP, FNP-BC is a board-certified Family Nurse Practitioner treating patients at our Phoenix School Based Health Center in Washington Heights. She speaks English and Spanish.
Alison is dedicated to keeping adolescents healthy so they can take advantage of educational opportunities.
Dr. Martin completed a Bachelor of Arts degree in English at Trinity College and an Associates degree in Nursing at Norwalk Community College. She completed her Doctorate in Nursing at Quinnipiac University. She graduated from the CHN NP Fellowship Program.
She has previously worked at AmeriCares providing care to immigrants and the underinsured.

Kelly Horn, DO is a board certified Family Physician treating patients of all ages. You can see her at CHN Jamaica in Queens.
Dr. Horn completed her undergraduate degree at SUNY Geneseo in Geneseo, NY, and her medical school at New York College of Osteopathic Medicine and medical residency in Family Medicine at Jamaica Hospital Medical Center.
Dr. Horn provides full-spectrum family medicine to patients of all ages. She has a special interest in women’s health, preventative care, and transgender health. Dr. Horn speaks conversational Spanish.
