Dr. Swetha Alugubelli, DDS, is a board-certified dentist serving patients at our Health Center in the South Bronx. She earned her Bachelor of Dental Surgery (BDS) in India before pursuing her Doctor of Dental Surgery (DDS) at Columbia University College of Dental Medicine in New York. Dr. Alugubelli further honed her skills by completing a General Practice Residency at Nassau University Medical Center in East Meadow, New York.
Emily Zhao is a board-certified nurse practitioner who works as family nurse practitioner. She treats patients at our Harlem location. Emily Zhao, FNP-BC completed an undergraduate degree in Biology at Boston College with a minor in Medical Humanities in 2019 and completed a doctorate degree in Family Practice at Columbia University School of Nursing in 2025 and her fellowship training at Community Healthcare Network. Emily Zhao is fluent in Mandarin as well.
Dr. Napolitano graduated from Rutgers Medical School in Newark, New Jersey, in 2007. He completed his residency at the Mount Sinai Downtown Family Medicine Program and worked as faculty in that program for much of his career. He also served as the Mount Sinai Family Medicine Department’s Inpatient Director from 2016 to 2021. Dr. Napolitano sees patients at our Lower East Side location. He has given many talks on Colorectal Cancer Screening and has developed CHN’s quality initiative programs. He has special interests in CQI, cancer screening, diabetes control, ambulatory management of heart failure, transitions of care, and the treatment of people living with HIV.
Chir Wei Stephanie Yuen, MD is a board-certified pediatrician and adult internal medicine doctor who specialized in HIV primary care and infectious diseases. She treats patients at our South Bronx Health Center. Dr Yuen completed an undergraduate degree in Biology and History at Rutgers University in 2015 and completed a Medical Degree at New Jersey Medical School in 2019. She completed her Med-Peds residency at Detroit Medical Center/Wayne State University in 2023 and Infectious Diseases Fellowship at Montefiore Medical Center in 2025. Dr Yuen is fluent in English and Cantonese.
Briana Cox, MD is a board certified Family Physician. She treats patients at our Williamsburg Health Center site in Brooklyn.
Dr. Cox completed her undergraduate and medical degrees through the combined BS/MD program at the Sophie Davis School of Biomedical Education and CUNY School of Medicine in Harlem. She then completed her Family Medicine residency at Montefiore Medical Center/Albert Einstein in the Bronx.
Sharon Ann Taylor-Smalls, DNP, CNM is a Certified Nurse Midwife. She treats patients at our Washington Heights location. Dr. Taylor-Smalls completed an undergraduate degree in Nursing at Columbia University in 2004 and her Master’s Degree in Midwifery at Columbia University in 2006. She completed her Doctorate of Nursing Practice at SUNY Stony Brook in 2022.
Dr. Taylor-Smalls is a full-time Clinical Assistant Professor at NYU Rory Meyers College of Nursing and has done research on relationship-based care.
Dr. Laurence Cloutier-Champagne, DPM is a board-certified Podiatrist treating patients at Harlem and Washington Heights.
Dr. Champagne received her Doctorate from the New York College of Podiatric Medicine in 2009. She completed her 3 years surgical residency program a at Interfaith Medical Center in Brooklyn. She is board certified with the American Board of Podiatric Medicine.
Dr. Champagne treats patients of all ages. She speaks English and French.
Nadine Osorio, RD, CDN, is a board-certified Registered Dietitian who works as a Nutritionist at our Williamsburg Center. She speaks English and some Spanish. Nadine completed her Master’s in Nutrition at Hunter College, CUNY, in 2024.

Nicole is a board-certified Registered Dietitian-Nutritionist. She sees patients at our Washington Heights Health Center in Manhattan. Nicole completed her graduate degree at NYU and her dietetic internship at the James J. Peters Bronx VA Medical Center. She received her BA in Sociology from the University of Washington.
