Dr. Anjali Prakash is a board-certified family medicine physician who works as the medical director at CHN Long Island City and a faculty physician with the CHN residency program. She completed her medical degree at Western Sydney University in 2015 and completed her residency with the Royal Australian College of General Practitioners in 2020. Dr. Prakash completed the NYC Homeless Healthcare Fellowship at Montefiore Einstein in 2026.

Ellie Lobovits, DNP, CNM is a certified nurse midwife. She treats patients at our Crown Heights, East New York and Sutphin locations. Ellie completed an undergraduate degree at the University of Pennsylvania in 2002, and completed both her Masters of Nursing (2023) and Doctorate of Nursing Practice (2025) at Columbia University. She is fluent in English and proficient in Spanish.

Helen Jin is a board-certified family medicine physician who sees patients of all ages at our Long Island City location. She is certified in medical Spanish and speaks conversational Mandarin Chinese.

She completed her medical degree from Boston University School of Medicine and graduated Lawrence Family Medicine Residency. Her interest is working with patients from all different backgrounds and helping them achieve their health goals.

Mikayla Chouljian is a Certified Nurse Midwife who works as a Midwife. She treats patients at our Long Island City location. Mikayla Chouljian completed an undergraduate degree in Cleveland at Case Western Reserve University in 2020 and completed her Masters Degree in Certified Nurse Midwife at Case Western Reserve University in 2024.

Jacqueline A. Berenson, MD, who works as the Director of Psychiatry (she/her) treats patients at our Washington Heights and Harlem sites.  Dr. Berenson completed an undergraduate degree in Behavioral Neuroscience in 1999 then did graduate research in Immunopathology before completing her MD in 2004, all at Memorial University of Newfoundland, Canada. Dr. Berenson then trained in General Adult Psychiatry at the teaching hospitals of Harvard University in Boston, MA; she then did a research Fellowship at New York State Psychiatric Institute and Columbia University in Medicine, Ethics and the Law (2011) and then a second fellowship in Forensic Psychiatry at the combined Columbia/Cornell training program (2012). She has been in practice in New York State since she finished training.

John O’Brien is a board certified psychiatric mental health nurse practitioner who works as a psychiatric nurse practitioner. He treats patients at our Crown Heights, Williamsburg, and Lower East Side locations. John completed his undergraduate degree in nursing at the University of Delaware in 2020 and completed his Master of Science in Nursing in psychiatry at the University of Pennsylvania in 2023. He completed a nursing fellowship at CHN in 2025. During the fellowship he helped implement a quality improvement project that educated staff on suicidality and improved assessment and documentation for CHN patient’s in crisis. John’s specific interests include substance use disorders and suicide prevention.

Prof. Mallika Govindan, MD, AAHIVS, is a Family Physician who works as a Faculty Physician at CHN. She treats patients at our Long Island City Health Center. Prof. Govindan completed an undergraduate degree in Biology and Gender Studies at Wellesley College in 2014 and completed her Medical Degree at Albert Einstein College of Medicine in 2020. She completed her residency in Family Medicine at the Institute for Family Health (Harlem) and Mount Sinai Hospital in 2023. She is fluent in English.

Shirley is a Family Nurse Practitioner who treats patients at both our Washington Heights Center and Pheonix, one of our School-Based Health Centers. She completed her undergraduate degree in nursing at Chamberlain University in 2019 and completed her nurse practitioner degree in family medicine at Pace University in 2022. She also completed Community Healthcare Center’s NP Fellowship. Shirley is fluent in English and Spanish.

Tanzilan Nahar is a board-certified family medicine physician. She treats patients at our center in Jamaica, Queens. Dr. Nahar completed an undergraduate degree in biology at St. John’s University and completed her medical degree at University of Health Sciences Antigua. She completed her residency at Conemaugh Memorial Medical Center in 2025. Dr. Nahar is fluent in English and Bangla.
