Madison Meek, MD is a board-certified family medicine doctor who works as a staff physician. She treats patients at our Lower East Side location. Dr. Meek completed an undergraduate degree in Psychology at Texas A&M University in 2017 and earned her Doctor of Medicine (MD) degree at University of Texas Health Science Center at Houston (McGovern Medical School) in 2022 with a scholarly concentration in Medical Humanities and Ethics. She completed her residency in Family Medicine at Kaiser Permanente San Jose in 2025.

Dr. Meek has a particular passion for women’s health, with experience in routine gynecologic care, contraceptive counseling, and office-based procedures, including IUD and Nexplanon insertions and removals. Her clinical interests also include adolescent medicine and preventive care. She is dedicated to creating a welcoming and supportive environment where patients feel comfortable discussing their health concerns and empowered to make informed decisions about their care.

Ciara Mottier DNP, MPH, FNP-BC is a Board-certified Family Nurse Practitioner. She treats patients at our Crown Heights health center. Ciara completed an undergraduate degree in Public Health at the University of Massachusetts Amherst, a Master of Public Health in Health Management and Policy at Oregon State University and completed her Nursing degrees at Columbia University. She completed her Family Nurse Practitioner fellowship at CHN in 2026.

Mirella Salvatore, MD FISDA is a double-board certified physician in Internal Medicine and Infectious Diseases. She treats adult patients at our South Bronx center clinic, where she is also the Medical Director.

Dr. Salvatore completed her Medical Degree Summa Cum Laude from the Catholic University in Rome, Italy and completed her Internal Medicine residency and Infectious Diseases fellowship at Mount Sinai School of Medicine. She also completed Infectious disease post-doctoral training in neurovirology at the University of California Irvine and at Mount Sinai studying influenza.

Dr. Salvatore has a special interest in HIV medicine including PrEP and PEP, treatment of hepatitis B and hepatitis C, travel medicine (consultation and vaccines before travel) and LGBTQ+ care.

Dr. Salvatore has also a long research and academic background. She is a Fellow of the Infectious Diseases Society of America (IDSA) and serves in leadership roles within regional and national infectious disease organizations. A list of her publications can be found here.

Dary Korng, RDH is a registered dental hygienist. She treats patients at our Jamaica Health Center in Queens, NY. She received her bachelor’s degree from New York University College of Dentistry.

Dr. Rashanique Duplessis is a General Dentist who works as a Staff Dentist. She treats patients at our South Bronx health center. She completed her Doctorate of Dental Surgery at Meharry Medical College School of Dentistry in 2022. She completed her residency at the BronxCare Health System. She is fluent in English.

Ashley Chisom Okuagu, DO is a doctor of osteopathic medicine who works as a Staff Physician. She treats patients at our Sutphin Blvd. site. She completed an undergraduate degree in physiology at The University of Arizona in 2018 and completed her Medical Degree in osteopathic medicine at Kansas City University in 2023. She completed her residency at University of Arizona College of Medicine – Phoenix Family Medicine Residency in 2026. She is fluent in English and proficient in Medical Spanish.

Dr. Okuagu has completed research and attained several publications in sleep health, as well as participated in quality improvement research and the impact of inclusive hair care in the tertiary hospital setting. Special interests include women’s/reproductive health, preventative medicine, global health, and health equity.

Laurie Schwartz-Moser is a board certified Midwife. She treats patients at our Jamaica Health Center in Queens. She completed her graduate degree at State University of New York, Health Science Center.

Dr. Anjali Prakash is a board-certified family medicine physician who works as the medical director at CHN Long Island City and a faculty physician with the CHN residency program. She completed her medical degree at Western Sydney University in 2015 and completed her residency with the Royal Australian College of General Practitioners in 2020. Dr. Prakash completed the NYC Homeless Healthcare Fellowship at Montefiore Einstein in 2026.

Ellie Lobovits, DNP, CNM is a certified nurse midwife. She treats patients at our Crown Heights, East New York and Sutphin locations. Ellie completed an undergraduate degree at the University of Pennsylvania in 2002, and completed both her Masters of Nursing (2023) and Doctorate of Nursing Practice (2025) at Columbia University. She is fluent in English and proficient in Spanish.
