Dr. Pushpa Yadav, MD, is a board-certified family medicine physician who works as a faculty physician at Community Healthcare Network. She treats patients at our Long Island City location. Dr. Yadav is from Nepal and completed her medical degree from People’s Friendship University of Russia in 2002 and completed her family medicine residency at BronxCare Health System in 2024. Dr. Yadav is fluent in English, Nepali, Hindi, and Russian. Prior to her family medicine residency journey in the Bronx, she has worked for more than 10 years as a primary care physician across Nepal, American Samoa, and Marshall Island. She enjoys traveling, spending time with family and friends, and listening to music.
Shirley Candelario, MSN, FNP-BC, CDCES, is a board-certified Family Nurse Practitioner who works as a Medical Director – FNP. She treats patients at our Washington Heights Health Center. Shirley Candelario FNP completed an undergraduate degree in Latin American Literature at NYU in 2003, then returned to school to pursue a nursing degree at Monroe College in 2011. While working as an RN, she graduated with a Master’s of Science in Nursing from Columbia University in 2017. She is fluent in Spanish and is also a Certified Diabetes Care and Education Specialist (CDCES).
Dr. Innes Tounkel is a board-certified family medicine physician who works as a Faculty Physician – Family Medicine. She treats patients at our Jamaica and Long Island City locations. Dr. Tounkel completed her Medical Degree at New York Medical College in 2021. She completed her family medicine residency at Jefferson Family and Community Medicine in 2024. Dr. Tounkel is fluent in Russian and English and she is working on improving her conversational Spanish skills. She is particularly interested in reproductive health and completed a Reproductive Health Care and Advocacy fellowship through the Reproductive Health Access Project and the Institute for Family Health in 2025.
Bianca Lozano is a Registered Dental Hygienist with a Bachelor of Science degree in Dental Hygiene. She treats patients at our South Bronx Health Center in the Bronx. Bianca graduated from New York University College of Dentistry and is licensed to administer local anesthesia.
Sasha Lawson, DNP, APRN, PMHNP-BC is a Certified Psychiatric Mental Health Nurse Practitioner who works as a Psychiatric Nurse Practitioner. She treats patients at our Jamaica location. Sasha Lawson, DNP, APRN, PMHNP-BC completed an undergraduate degree in Nursing at Long Island University- Brooklyn campus in 2006. Sasha Lawson completed her post graduate certification degree in psychiatry and doctorate degree at University of Massachusetts Global in 2023. Sasha Lawson is fluent in English.
Dr. Swetha Alugubelli, DDS, is a board-certified dentist serving patients at our Health Center in the South Bronx. She earned her Bachelor of Dental Surgery (BDS) in India before pursuing her Doctor of Dental Surgery (DDS) at Columbia University College of Dental Medicine in New York. Dr. Alugubelli further honed her skills by completing a General Practice Residency at Nassau University Medical Center in East Meadow, New York.
Isadora (Dory) Kagan DNP, FNP-BC is a licensed Family Nurse Practitioner who works as a Family Nurse Practitioner. She treats patients at our Crown Heights Location. Dory Kagan DNP, FNP-BC completed an undergraduate degree in Studio Art at Smith College in 2011, and completed both her Masters of Nursing (2022) and Doctorate of Nursing Practice (2025) at Columbia University. She is fluent in English and proficient in Spanish.
Emily Zhao is a board-certified nurse practitioner who works as family nurse practitioner. She treats patients at our Harlem location. Emily Zhao, FNP-BC completed an undergraduate degree in Biology at Boston College with a minor in Medical Humanities in 2019 and completed a doctorate degree in Family Practice at Columbia University School of Nursing in 2025 and her fellowship training at Community Healthcare Network. Emily Zhao is fluent in Mandarin as well.
Dr. Napolitano graduated from Rutgers Medical School in Newark, New Jersey, in 2007. He completed his residency at the Mount Sinai Downtown Family Medicine Program and worked as faculty in that program for much of his career. He also served as the Mount Sinai Family Medicine Department’s Inpatient Director from 2016 to 2021. Dr. Napolitano sees patients at our Lower East Side location. He has given many talks on Colorectal Cancer Screening and has developed CHN’s quality initiative programs. He has special interests in CQI, cancer screening, diabetes control, ambulatory management of heart failure, transitions of care, and the treatment of people living with HIV.
