How to pay for care?
If you do not have health insurance, we can create a payment plan that fits your budget and healthcare needs. You will make payments based on a sliding fee scale.
What is a sliding fee scale?
If you have insurance or do not have insurance, how much you pay for a service is based on how much money you make. If you make less money, you will pay less for care.
What extra costs are there?
Extra costs depend on what you need during your visit and what your health insurance covers. If you don’t have health insurance, there may be extra fees for lab work and some medicines. If you have any questions about extra fees, please ask when you are setting up your visit.

Changes to SNAP, the Essential Plan, and Medicaid
New federal requirements for enrollment in Medicaid, the Essential Plan, and the Supplemental Nutrition Assistance Program are beginning in New York State. It is important for individuals who are enrolled in Medicaid, the Essential Plan, and/or SNAP to check your mail for notices from New York state. Please verify and update your address, phone number, or email per program to ensure you can be reached.
Changes to SNAP as of March 1, 2026
As of March 1, 2026, new rules require certain SNAP (food stamp) recipients to prove they are working, volunteering, or in a school for a specific number of hours each month in order to keep their monthly benefits. The rule applies to certain adults aged 18 to 64. Exemptions include individuals who live with a child under 14 years old, individuals who provide care for a person who cannot care for themselves, and/or individuals with mental or physical conditions that prevent them from working. Changes to Essential Plan as of July 1, 2026
On July 1, New York Essential Plan health insurance was terminated for individuals who earn 200% to 250% of the 2026 federal poverty line. Over 450,000 individuals are expected to lose coverage. Individuals formerly receiving Essential Plan coverage may transition to private insurance policies, or Qualified Health Plans, through the New York State of Health marketplace. If you have questions about your coverage or need support to enroll in a Qualified Health Plan, please contact the Center Manager at your CHN Health Center.
Changes to Medicaid Starting January 1, 2027
Starting January 1, 2027, certain adult Medicaid enrollees aged 19 to 64 must meet work and community engagement (W&CE) requirements to maintain Medicaid coverage. The W&CE requirements call for 80 hours per month of work, job training, education, or community service. Individuals affected by this rule should expect to receive notice from NY State of Health via mail, phone, or email in August or September. Those who are exempt include children, older adults, pregnant people, certain caregivers, and people with disabilities or serious health conditions.
