HIV and Hepatitis Care

We provide comprehensive, compassionate support for individuals living with HIV and Hepatitis. We provide personalized care plans, including testing, diagnosis, treatment, and ongoing management, all designed to improve health outcomes and enhance quality of life. Our multidisciplinary team ensures patients receive expert guidance on medication, lifestyle adjustments, and preventive measures, empowering them to take control of their health while minimizing the risk of transmission. HIV Care

HIV (Human Immunodeficiency Virus) is a virus that attacks the immune system, weakening the body’s ability to fight infections and diseases. CHN can help with:
  • Non-medical case management
  • Counseling and other helpful services
  • Access to empowerment groups

Hepatitis (Liver Disease) Care

Hepatitis is an inflammation of the liver, commonly caused by viral infections like hepatitis A, B, or C. It can lead to symptoms such as jaundice, fatigue, and liver damage if left untreated. CHN can help with:

  • Hepatitis testing and diagnosing
  • Vaccinations for hepatitis A and hepatitis B
  • Medical care for hepatitis
  • Non-medical case management
  • Counseling and other helpful services
  • Access to empowerment groups

Health Centers Offering HIV and Hepatitis Care Services

All Community Healthcare Network health centers and offices will be open on Tuesday, February 24, 2026. Our mobile health clinics will not be operational due to weather conditions.
