Prenatal Care

At CHN, you are the most important member of your care team. We work with you and support you to stay healthy during and after your pregnancy. We welcome all people who are pregnant, including cisgender, transgender, and nonbinary individuals. Our team treats everyone with respect and care to support your needs and goals.

Your care team may include a doctor, midwife, or nurse practitioner, along with a prenatal coordinator, nutritionist, and social worker. Your team works together to help you stay healthy and support your baby’s growth. You can also meet your baby’s pediatric provider before you give birth, so you feel connected and cared for from the start.

Our Services

Regular Pregnancy Check-Ups

These visits help us check your health and your baby’s growth.

  • Every 4 weeks until 28 weeks
  • Every 2 weeks from 29–35 weeks
  • Every week from 36 weeks until delivery

Support and Education

  • What to expect leading up to and including delivery
  • Preparing for your baby (safe sleep, feeding your baby)
  • Mental health screening and counseling
  • Nutrition support and help enrolling in WIC
  • Help making newborn appointments with a pediatric provider
  • Baby feeding help, including breastfeeding education and lactation
  • Care and support for you and your baby after delivery

Connections to Community Programs

We can connect you to trusted programs that support you outside the clinic, such as:

  • Doulas: trained, non-medical helpers who can support you during pregnancy, labor, and after birth
  • Nurse-Family Partnership: nurses who visit you at home during pregnancy and your baby’s first years
  • Family Wellness Suite and local community partners: programs that can help with parenting, feeding, baby supplies, stress support, and more

These services are meant to make your life easier, lower stress, and give you a strong circle of care.

Referrals

  • Referrals for anatomy scans and ultrasounds
  • If you need a specialist or higher‑level care, we will help you get connected quickly and easily

What to Expect When You Choose Us for Your Care

We want you to get care as soon as you need it. When you schedule your first appointment, you may be offered a quick visit to check your blood pressure, do basic lab tests, give you prenatal vitamins, and answer your early questions.

This helps you get started right away. We will then schedule a longer first prenatal visit where we can learn more about you and your needs.

What to Expect at Your First Prenatal Visit

Your first prenatal visit is an important time for us to get to know you. At this visit, we will:

  • Talk about your health history and answer any questions you have
  • Check your weight, blood pressure, and overall health
  • Do basic lab tests, like blood and urine tests
  • Talk about your due date
  • Discuss any symptoms
  • Plan your next visits
  • Share ways to stay healthy during your pregnancy
  • Connect you with any services or programs you are interested in

Your first visit may take longer than a regular checkup because we want to learn about you, understand your needs, and make sure you feel supported from the very start. You are welcome to bring a partner, family member, or friend for support.

After Your Baby Is Born

Your care continues after delivery. We stay with you through the postpartum period and into early parenthood.

We offer:

  • Postpartum checkups to support your healing
  • Help with breastfeeding and infant feeding
  • Mental health support
  • Tips for bonding, safe sleep, and newborn care
  • Easy scheduling with your baby’s pediatric provider at CHN

We believe postpartum care is just as important as prenatal care. You deserve support every step of the way.

We want you to feel safe, informed, and cared for throughout pregnancy, birth, and after your baby arrives. You share your goals, we provide the care and support to help you reach them.
