Update: Parent Portal Access

Dear Parent or Guardian,

We would like to inform you that as of September 1, 2026, access to your child’s Healow account through Community Healthcare Network (CHN) will be limited to viewing immunizations and referrals for all patients ages 12 and older. This change is required to comply with the Health Insurance Portability and Accountability Act (HIPAA) and New York State laws that protect minors’ rights to confidentiality.

You may still request your child’s medical records by calling or visiting your health center.

If you have any questions or would like more information, please speak with our front desk staff during your next visit or go to: chnnyc.info/nylaw

Thank you,
Community Healthcare Network (CHN)
