Community Healthcare Network works hard to provide excellent care to patients. Our Patient Experience Star Rating reflects the results from the 2023 CAHPS Survey, a standardized questionnaire sent to a random group of verified patients and distributed by a third-party vendor, forHealth Consulting. The survey evaluates the quality of services and care that patients received throughout the entirety of their visits.
All Community Healthcare Network health centers and offices will be open on Tuesday, February 24, 2026. Our mobile health clinics will not be operational due to weather conditions.
Dear Parent or Guardian,
We would like to inform you that as of September 1, 2026, access to your child’s Healow account through Community Healthcare Network (CHN) will be limited to viewing immunizations and referrals for all patients ages 12 and older. This change is required to comply with the Health Insurance Portability and Accountability Act (HIPAA) and New York State laws that protect minors’ rights to confidentiality.
You may still request your child’s medical records by calling or visiting your health center.
