Geriatric Care

CHN offers a range of specialized care for older adults. We understand that seniors have unique medical needs, and we focus on creating a caring place that keeps them healthy. Adults who are 65 years old or more can schedule an appointment to meet our geriatrician (a doctor who focuses on people over 65). Our doctor is available on Tuesdays, from 9 am to 12 pm, at our Long Island City Health Center.

Health Centers Offering Geriatric Care Services

All Community Healthcare Network health centers and offices will be open on Tuesday, February 24, 2026. Our mobile health clinics will not be operational due to weather conditions.

Update: Parent Portal Access

Dear Parent or Guardian,

We would like to inform you that as of September 1, 2026, access to your child’s Healow account through Community Healthcare Network (CHN) will be limited to viewing immunizations and referrals for all patients ages 12 and older. This change is required to comply with the Health Insurance Portability and Accountability Act (HIPAA) and New York State laws that protect minors’ rights to confidentiality.

You may still request your child’s medical records by calling or visiting your health center.
