HIV Care
Community Healthcare Network is NYC's premier community provider for medical and social services for those living with HIV and at risk for HIV. The network has been intensely involved in planning, developing, and providing HIV/AIDS services since HIV first appeared in the communities we serve. CHN led the way as the first agency in the U.S. to incorporate HIV services, such as counseling and testing, into Family Planning services and the first in New York City to implement COBRA Case Management services for its HIV patients. Its COBRA program is one of New York’s largest, and CHN is the first agency in New York that offered urine-based HIV testing. CHN is a New York State Department of Health AIDS Institute Authorized Training Agency. CHN helps enroll HIV+ patients into the NYS AIDS Drug Assistance Program through its own Access to Healthcare Program. The New York state-funded AIDS Drug Assistance Program helps to cover the health care costs of HIV+ patients not covered by Medicaid.

Assists patients with their treatment through education, home visits, conferences with patients’ providers, and follows up after missed medical appointments. - Nutrition and weight loss prevention therapy

- Women's Supportive Services (Queens Health Center, Dr. Betty Shabazz Health Center)
- Dental Care
- Prenatal Care
- Transgender Program
- Caribbean Access Initiative (Caribbean House Health Center)
* Many of these services are available to both HIV+ and HIV- patients, but are listed here to recognize the special medical attention required in many areas of primary care when a patient is HIV+.
Our HIV Team
Our HIV Care team is overseen by Dr. Yvette Walker, MD, MPH who has extensive experience in Infectious Diseases and developing, expanding and overseeing HIV Care programs. She is joined by Associate Medical Director of HIV Clinical Services, Dr. Elsa Escalera, and the Assistant Vice President for HIV Programs, Dr. Luis Freddy Molano. CHN’s HIV Team will provide you seamless, professional and confidential care by coordinating each patient’s total care.
