About Appointments

Making an Appointment

Appointments can be made in advance by calling the health center you would like to visit Monday-Friday. The appointment scheduler will ask you a few questions to get the process started. If the medical problem is an emergency you will be directed to the nearest emergency room.

Our health centers offer a wide range of programs and services for people of all ages. In most cases, care can be given at all our health centers.

To make scheduling an appointment as easy as possible you might want to gather the following information beforehand:

If you have insurance, have the card available as it contains all the necessary information we need. If you do not have health insurance you will speak with a financial advisor to determine fees for services based on your income.

What to Bring with You

Everyone who comes to our health centers for the first time should be prepared to bring the following documents with them to their appointment.

This can be any official picture identification card, such as a driver’s license or work ID.

These documents are used to confirm that you live at a certain address, such as a Con Edison utility bill or telephone bill.

This is used by our financial counselors to help people get on an insurance program or payment plan if necessary. Examples are a pay stub, bank statement or letter from your employer.

Bring a list (or the bottles) of all the medications, supplements, vitamins, herbs or over-the-counter remedies you are currently taking to show to the doctor.

Bring any information about past or present health problems, surgeries, family history of disease and children’s immunization records.

NOTE: If you don’t have the above documents don’t worry, you will still be able to receive care. Without proof of income you will be asked to sign a Financial Self-Declaration Form that certifies your income for financial assistance. This helps our financial administrator determine eligibility for a discounted fee schedule.

Appointment Availability

In an effort to accommodate patients’ schedules, each of our health centers has appointments available throughout the day including some evenings and weekends. Please let our appointment scheduler know if you need a particular time for your appointment. Our scheduling department makes every effort to get patients a timely appointment.

Walk-in appointments are available on a case-by-case basis. We highly recommend that you schedule an appointment in advance to ensure you can be seen by one of our providers at your convenience, and to minimize wait time.
