Dear CHN Community Member,

 

At Community Healthcare Network (CHN), it is very important to us to keep all staff and patients safe. We are taking the Coronavirus (also known as COVID-19) very seriously.

 

CHN is taking extra care and following the Centers for Disease Control (CDC) guidelines to provide the safest environment possible. Along with lots of hand washing and proper hygiene practices, all counters and surfaces will be cleaned throughout the day across all our health centers. All patients that visit our health centers are being screened for Coronavirus symptoms and masks are being given out to help limit exposure.

 

We have also made some changes in how our centers are being run at this time. We want to make you aware of these changes:

  • For your safety, all non-essential visits will be done through telephone visits. We are also open for urgent, walk-in visits. Please call our Contact Center at 1-866-246-8259, to set-up a telephone visit. Please check our website for a full list of services that will be offered by telephone visit.
  • For all visits that have already been scheduled, we will contact you in order to tell you if your visit will become a telephone visit, or if you will still need to be seen at the Health Center.
  • All CHN centers will be open for urgent walk-ins, along with essential scheduled visits. Our centers will be open Monday through Friday from 9 a.m. to 5 p.m. All of our centers will be closed on Saturdays and Sundays. Our Mobile Health Clinics will continue to run on their own schedules, which will be posted on our social media pages.
  • Therapy and psychiatry visits will all be telephone visits, except where an in-person visit is medically needed.
  • All Dental and Eye visits will be postponed, unless medically needed.
  • During your telephone visit, your provider may decide that you need to visit a Center. In this case, you will be given instructions on how to do so safely. Please only come in to a Center for a visit if told to do so.
  • At the advice of the CDC and the Department of Health, if you are feeling sick, the best thing to do is stay home. If you have these symptoms related to COVID-19 (fever, cough, shortness of breath), do not come into our health centers. Instead, please call our nurse at 1-866-246-8259 for next steps. We are not testing for COVID-19 at our centers.

We want to remind you that, while this is an uncertain time, most people who get this virus will see mild symptoms and will get better. We want to help stop the spread.
