Dr. Sachin Jain, MD, MPH is a board certified internal medicine and infectious diseases specialist. He treats patients part-time at CHN Long Island City. He is fluent in English, Spanish, and Hindi. Dr. Jain completed his undergraduate degree at the University of Illinois and his medical degree at Rush Medical College. He completed his residency at Yale-New Haven Hospital and fellowship at Beth Israel Deaconess Medical Center/ Harvard Medical School.
Dr. Jain has been designing and implementing healthcare delivery solutions in various settings over the past decade, domestically and abroad. He co-founded a cataract surgery program in rural Tanzania, a home-based HIV counseling and testing program in a slum in Nairobi, a clinic for newly resettled refugees in New Haven, and expanded HIV prevention and sexual health services for at-risk women and men in the Bronx. When he is not seeing patients, he works at the New York City Department of Health and Mental Hygiene, where he leads clinical strategy, informatics, and evaluation of practice transformation and value-based care initiatives as Executive Director of the Primary Care Information Project. In his role, he co-leads the ThriveNYC Mental Health Service Corps program, which integrates behavioral health services into primary care and behavioral health settings. He is a national leader in HIV post-exposure prophylaxis research and has published his work in several leading Infectious Diseases and HIV journals, which has been cited in CDC and WHO guidelines. He maintains a faculty appointment at Albert Einstein College of Medicine.
