Case Management
This service addresses the medical and psychosocial needs of our clients by providing family-centered Case Management. The program is designed to improve our clients’ and their families’ access to a full range of health and supportive services, and to make sure the "system" is responsive to their needs. The client is assisted in developing a service plan, which will meet their needs. The CHN Case Management team acts as a client advocate—we monitor progress in obtaining services and make necessary adjustments to the service plan as the client’s resources and needs change over time. Because some CHN clients face enormous barriers, they may require frequent contacts, home visitation and community-based follow. Unique to Community Healthcare Network is our connection to the many resources housed in our own community based healthcare centers, and our focus on the needs of women and children. Case Management is also available for patients who do not qualify for Medicaid. Two of our programs work specifically with the HIV+ patient population: COBRA Case Management and Women’s Supportive Services.
In addition to being offered at all CHN health centers, Case Management is also provided at 1916 Park Avenue.
Social Work
Trained social workers are available at every center to provide confidential counseling, crisis intervention and service referrals. Social workers meet with every adolescent, pregnant and HIV patient, in addition to others who are referred or request to meet with the center social worker.
Adolescent Services
CHN’s adolescent medical and social services are provided through our specialized Teens P.A.C.T program.
