
CHN believes that a key part of comprehensive treatment is addressing the psychosocial needs of our clients. The social services program is designed to improve our clients’ and their families’ access to a full range of health and supportive services, and to make sure that they receive all the support they need to lead healthy productive lives. Most importantly, social services play a vital role in coordination of care. We have many available resources through our centers for this purpose.
Social Workers : CHN’s trained social workers are available at every center to provide confidential counseling, crisis intervention and service referrals. Social workers meet with all pregnant patients, adolescent patients and HIV-positive patients, as well as anyone who requests social services or is referred by a healthcare provider. Every patient receives an appropriate service plan to meet his or her individual needs. Adolescent medical and social services are provided through our specialized Teens P.A.C.T program.
Case Managers : CHN’s Case Management team acts as a client advocate to monitor their progress in obtaining services, and to make any necessary adjustments to the service plan as the client’s resources and needs change over time. Because some clients face enormous barriers, they may require frequent contacts, home visitation and community-based follow-up. Two of our case management programs, COBRA Case Management and Women’s Supportive Services, work specifically with the HIV-positive community.
