Our Primary Care Health Services

Primary medical care is provided by family physicians, internists, nurse practitioners and physician assistants at all CHN centers. Referrals for specialty services are made to hospitals within the communities where patients reside. We provide services to prevent, diagnose, and treat the most prevailing health conditions affecting our neighboring communities. We emphasize holistic care through high quality medical services that coordinate care to treat a person’s whole mind and body.
