
People living with HIV face enormous barriers to effective care. CHN centers have many resources and services available to help HIV-positive patients receive optimum care such as workshops, support groups and on-site mental health services.
CHN offers two case management programs for HIV-positive individuals, overseen by the Social Work Department: the COBRA program for those who are Medicaid-eligible and the Women’s Supportive Services Program (WSS) for women who are not Medicaid-eligible.
COBRA Case Management (Also known as The Community Follow-up Program) is an initiative by the New York State Department of Health AIDS Institute. CHN was one of the first agencies in New York City to implement COBRA services.
The COBRA program is designed to coordinate care and improve our clients’ and their families’ continued access to a full range of health and supportive services, and to make sure patients remain in the healthcare system. CHN’s Case Management Team acts as a client advocate, making necessary adjustments as the client’s resources and needs change over time.
Support services are offered to all patients, whether or not they are Medicaid-eligible. We also offer transportation reimbursements to make travel to our health centers even easier.
