Health Homes: Frequently Asked Questions

Do I have to join a Health Home?
No. But we strongly encourage you to join. The Health Home program helps you with all of your health and social services needs.

How was I chosen for the Health Home program?
You were chosen for the program based on your health needs and the healthcare providers that you see.

Is there a fee for joining a Health Home?
No. Health Home services are free.

Do I need to fill out an application?
No. You only need to fill out a consent form. Once you sign the consent form, all of your healthcare providers can work together to give you the best care possible.

Will I be seeing a new doctor?
In most cases, you will keep seeing all the same healthcare providers that you see now.

Can I change my Health Home?
Yes. Call your health insurance plan or your Health Home to let them know you want to change.

I don’t want to be in a Health Home. How can I get out of this program?
You do not have to join if you do not want to. If you don’t want to be in a Health Home, call your Health Home within 30 days of getting your letter in the mail and let them know.

If I am not in a Health Home, will my care change?
If you are not in a Health Home, you will still get the services you have been getting. But if you are in a care management program, call your care manager to see if your care will change.

If I join a Health Home, what do I do if I have an emergency?
Call 911 or go to the hospital.

Will I get transportation to and from my Health Home?
Your care manager will help you get to and from doctors’ visits if you need help.

Why was I matched to this Health Home?
You were matched to this Health Home based on where you live and which providers you see.

What is a care manager?
A care manager is a trained person who helps you get the health care and social services you need.

When will I find out who my care manager is?
Once you sign up for a Health Home, your care manager will call you.
