Our Health Education Services

Health education is offered at every medical visit…

At the centers, education takes place through one-on-one counseling and group workshops. Our medical, nutrition, health education and social work staff are trained to provide individualized education on a variety of health topics. All CHN staff are trained to provide education that is based in health literacy principles. Our philosophy is to provide our patients with health information in a way that is easy for them to understand, no medical jargon. Just plain, simple language  so they can make informed decisions about their health care and leave the health center knowing how to care for themselves at home. Our clinical staff take time to answer patient questions and offer advice and solutions.

Health education is offered outside of the clinic…

CHN provides education in a variety of settings outside the health clinics.

  • Workshops: Staff give workshops at a variety of community-based organizations, including schools, faith-based organizations, shelters, soup kitchens and substance abuse treatment centers.
  • Mobile Clinic: Further education is provided through our mobile van.
  • Health Fairs: CHN regularly participates in community health fairs, offering education and health screenings at no cost.
  • Outreach: CHN’s outreach team conducts ongoing street outreach, disseminating information and escorting patients to CHN centers for services.

Specialized Programs:

Family Planning and Reproductive Health Education

Patients seeking family planning services at CHN can meet with a family planning health educator (or social worker if needed) to discuss their reproductive health concerns. Each session is targeted to the patient’s specific needs and concerns.

Workshops: Comprehensive sexuality education is offered to schools and community-based organizations at no cost to them. Popular topics include birth control methods, STIs/HIV, Puberty, Healthy Relationships, Body Image, Breast Health, Men’s Health and much more. Prenatal Health Education

Individualized education is provided to patients who are pregnant or planning for a future pregnancy. This education is provided by a team of skilled and experienced women’s health educators, social workers, nurses, and nutritionists. Group prenatal health education is offered and includes topics such as breastfeeding, labor and delivery, family planning, parenting and post-partum basics. For those looking for group prenatal health education, our Centering Pregnancy Program could be an excellent option.

Harlem Breast Health Awareness Initiative

Based out of our Helen B. Atkinson Health Center in Central Harlem, this initiative aims to educate our patients and the Harlem community about the importance of breast health. The program provides outreach, education and patient navigation services, promotes breast self-exam, clinical breast examination, mammography screening and immediate referrals for treatment of abnormal results.

Mammogram Mondays: Monthly mammograms are provided right outside the center on the first Monday of each month. The services offered during these “Mammogram Mondays” are for all women over the age of 40 regardless of their ability to pay.
