Services and Programs
CHN’s approach to patient care is holistic, taking into account not only a client’s physical well-being, but also their mental and social well-being. When a patient comes to our centers, an entire clinical team serves their needs. The main pillars of our patient care are (please click on the following links to learn more):

Below is a snapshot of some of our many services:
- Caribbean Access Initiative: Provides no-cost medical care, group support and HIV education to Caribbeans. (Caribbean House Health Center)
- "Centering" prenatal care: New model of prenatal care that provides individual medical care and group education/support to expectant mothers and their families ( Helen B. Atkinson Health Center).
- COBRA case management: This team helps HIV+ patients and their families with issues that include housing, legal referral & advocacy, public benefits, mental health, substance use, ACS issues and mandates, and other areas. (All sites)
- Diabetes self-management Program : Five-week group education program, combined with medical care, for patients with diabetes and their families. (Bronx Health Center)
- Heart Health Program : Five-week group education program—combined with medical care—for those with diabetes, high blood pressure, high cholesterol, and who are overweight/obese. (CABS Health Center)
- HIV Counseling & Testing : All CHN sites offer confidential no-cost HIV testing, counseling and referrals to CHN programs for appropriate care.
- Mental Health services : Individual, couples and family counseling, as well as group therapy, provided by licensed mental health therapists. Psychiatric services include medication management and evaluations by psychiatrists. (CABS Health Center, Caribbean House , Dr. Betty Shabazz)
- Reach Out and Read ®: Children receive a book during their annual “well visit.” Parents can learn from the pediatrician how to encourage their children to read. (Bronx Health Center and CABS Health Center)
- Teens P.A.C.T: Youth development program that provides sexual health education, leadership and training opportunities for its peer educators. Hosts after school rap sessions, provides confidential birth control advice and counseling. (Helen B. Atkinson Health Center and CABS Health Center)
- Transgender Program: Serves medical needs of transgender community in the Bronx with medical care, education, weekly support group meetings and workshops on topics that range from legal rights to HIV education. All walk-ins welcome! (Bronx Health Center)
- Treatment Adherence Program: Assists patients with their treatment through education, home visits, conferences with patients’ providers, and follow up after missed medical appointments. (All sites)
- Women’s Supportive Services: Provides support services for the HIV+ clients and their families. This includes crisis intervention, escort and transportation, childcare, emergency funds, weekly psycho-educational support groups, recreational activities, psychological counseling, legal services, and risk reduction.
