CHN Selected to Lead “Health Home” Medicaid Program in Brooklyn, Co-Lead in Bronx

Recognizing CHN’s excellence in providing care to underserved individuals in the City’s poorest neighborhoods, the New York State Department of Health has selected us to serve as a lead Health Home in Brooklyn and as co-lead in the Bronx.

The Health Home program will revolutionize the way we treat the approximately 975,000 chronically ill Medicaid patients in the state who do not have access to regular healthcare by providing them with better-coordinated and comprehensive care. The result will be a healthier population and an estimated $33.2 million cost savings to taxpayers in the first year.

Crain’s Health Pulse covered the announcement of the Health Homes selection, quoting our President and CEO Catherine Abate about the need for the program:

“Health care right now is not really a system,” said Catherine Abate, president of Community Healthcare Network, one of the new health home designees. The organization has been in existence since 1981 and has 11 federally qualified health centers. Currently, “these patients go from one silo to another,” she said, getting uncoordinated care from substance-abuse agencies, hospital clinics, and emergency rooms.

Beginning this year, CHN will coordinate with other health care providers and community-based organizations to oversee and ensure access for chronically ill Medicaid patients to a wide range of health and mental health services, comprehensive care management, transitional care, patient and family support, and referral to community and social support services. And we’ll do it by using health information technology to link services.

In our new role as a lead Health Home, we are better positioned than ever before to execute our mission to provide access to quality healthcare and social services for diverse populations in underserved communities throughout New York City.
