Job Opening: Case Manager Technician

Join CHN’s Team as a Case Manager Technician

The Case Manager Technician is an integral part of the Case Management team. Case Manager Technicians assist the Case Managers in the provision of services for the entire caseload. Their primary responsibility is in the implementation of the service plans as developed by the Case Manager. They function as liaisons between clients and the array of service providers involved in the clients’ care.

Location: Multiple Locations

Qualifications: Must demonstrate ability to work in a team structure, to organize and document information accordance with program standards. Effective verbal and written communication skills and strong interpersonal skills. Bachelors degree in Social Work or Human Service.s
